SGPGI OPD Online Registration Process
- Visit to the Official Website of the Sanjay Gandhi Postgraduate Institute of Medical Sciences Lucknow.
- Here, on the homepage, you will see a patient portal option. Click on that option.
- Then, two options will show up Online Registration and OPD Schedule.
- After that, you need to check out the OPD Timing and Doctor details.
- And then click on it.
- You need to click on Online registration if you wants to do registration.
- Then, you will be redirected to the SGPGIMS Patient Portal.
- In this portal, patient will be able to do online registration and can fill the form.
- On this page, a person will view the instructions.
- Read all these instructions carefully and click on I Have Read all the Above Instructions.
- Then click on continue.
- On your screen, the Patient Registration Form will show up.
SGPGI OPD Online Registration Process
- Select the date of online registration.
- Then, select department.
- then, you will see the name of the doctor which will be available on that date.
- you need to select your first middle and last name, provide your gender details, father’s name, marital status, date of birth etc.
- Then, select your occupation, email id, religion, mobile number.
- Then, provide details such as your address.
- Enter your house number, country, city, country, state, street name, pin code number.
- Enter nearest railway station, correspondence address.
- Then, provide contact person name, mobile number, phone number, relationship with contact person.
- Provide referral doctor, referral department and referral hospital details.
- Then patient identity information.
- Here, select the ID Proof such as Pan Card, Aadhar Card, Voter ID Card. Provide ID Card Number and name of the issue authority.
- After that, click on the Declaration then provide the captcha code.
- After that, click on the button of register and then you will be successfully register for the SGPGI OPD Appointment.
- Once the registration is done, then you will acquire a conformation message.

How to Check Schedule for SGPGIMS OPD?
- Visit to the SGPGI Official Website.
- On the homepage, you will see OPD Schedule Section. Just click on that.
- After that, the page will open.
- Here, you will see the search box, select your department and OPD Schedule Date.
- Click on Search. Then, In this section, the details regarding the doctors that are available in that week is mentioned.
SGPGI OPD Registration Imp Instructions
- The Patient will be able to do the OPD registration for next one month only not more then that.
- When you visit to register in your respective department, you need to check the working day of that department.
- Once you are done with the registration process then take a printout of your registration.
- Then, arrive on your scheduled OPD Date before 10:00 am. Visit to the counter of online registration. you will be asked to show your referral documents there with the online registration token.
